Несмотря на то, что иммунотерапия уже зарекомендовала себя в качестве эффективного подхода для лечения ПКР, предиктивные биомаркеры для рационального отбора пациентов по-прежнему не идентифицированы.</p><p>В данное исследование было включено 75 пациентов с метастатическим почечно-клеточным раком (мПКР), получивших ниволумаб во 2-й и последующих линиях терапии с 2015 по 2020 года в рамках программы расширенного доступа к препарату. Частота объективного ответа составила 21,3%. Медиана выживаемости без прогрессирования (ВБП) составила 5,5 мес. Медиана общей выживаемости (ОВ) не достигнута.</p><p>С целью анализа молекулярных биомаркеров, коррелирующих с ответом на иммунотерапевтическое лечение, мы провели полнотранскриптомное РНК-секвенирование 16 образцов (FFPE) у 15 пациентов с оценкой уровня экспрессии отдельных генов (PDCD1, CD274, CD8A, CD8B, CD4) и генных сигнатур (Angio, Teff, Myeloid Inflammation). Частота контроля заболевания не отличалась в подгруппах пациентов с высоким и низким значением экспрессии какой-либо из исследованных сигнатур, а дальнейший анализ методом главных компонент не позволил выявить кластеризацию пациентов с наличием и отсутствием объективного ответа.</p><p>Дальнейшие исследования на большей выборке пациентов помогут подтвердить или опровергнуть предиктивную роль выбранных для анализа биомаркеров в гетерогенной популяции больных ПКР.</p></abstract><trans-abstract xml:lang="en"><p>Abstract: Nivolumab was registered in Russia in December 2016 as a monotherapy for advanced renal cell carcinoma (RCC) and it remains a second‑line treatment choice for patients with disease progression after the use of tyrosine kinase inhibitors. Even though immunotherapy has already proven to be an effective approach for the treatment of RCC, predictive biomarkers for the rational selection of patients remain unidentified.</p><p>Seventy‑five patients with metastatic renal cell carcinoma (mRCC) who received nivolumab in the 2nd and subsequent lines of therapy from 2015 to 2020 under the expanded access program were enrolled in this study. The objective response rate was 21,3 %. Median progression‑free survival (PFS) was 5,5 months. Median overall survival (OS) was not reached.</p><p>To analyze molecular biomarkers correlated with the response to immunotherapeutic treatment, we performed whole‑transcriptome RNA sequencing of 16 samples (FFPE) in 15 patients with the assessment of the expression level for individual genes (PDCD1, CD274, CD8A, CD8B, CD4) and gene signatures (Angio, Teff, Myeloid Inflammation).</p><p>Disease control rates were not different for the subgroups of patients with high and low expression of any of the signatures examined, and further principal component analysis did not reveal clustering of patients with and without objective response.</p><p>Further studies on a larger sample of patients will help confirm or deny the predictive role of biomarkers selected for analysis in a heterogeneous population of RCC patients.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>почечно-клеточный рак</kwd><kwd>ниволумаб</kwd><kwd>анти-PD-1</kwd><kwd>иммунотерапия</kwd></kwd-group><kwd-group xml:lang="en"><kwd>renal cell carcinoma</kwd><kwd>nivolumab</kwd><kwd>anti‑PD‑1</kwd><kwd>immunotherapy</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Motzer R.
