Изучить показатели общей выживаемости и частоты интракраниальных рецидивов пациентов с метастазами в головной мозг после проведения радиохирургического лечения (РХ) на аппарате Гамма-нож в самостоятельном варианте лечения.</p></sec><sec><title>Методы</title><p>Методы. Проведён анализ результатов лечения 502 пациентов с метастазами в головной мозг (211 мужчин и 291 женщина). У большинства пациентов был диагноз рак молочной железы. У 259 пациентов (51,6%) были множественные (≥3) метастазов в головном мозге. Медиана суммарного объема и количества МГМ для каждого пациента составила 5,9 см3 (интервал 0,09–44,5) и 4 (интервал 1–36) соответственно.</p></sec><sec><title>Результаты</title><p>Результаты. Общая выживаемость пациентов в целом по группе составила 37,6% и 19,1% на сроке 12 и 24 месяца соответственно с медианой общей выживаемости после РХ лечения 8,6 месяцев (95% доверительный интервал (ДИ) – 7,0–10,0).</p><p>Развитие новых (дистантных) метастазов зарегистрировано у 164 (49,5%) пациентов. Медиана развития дистантного метастазирования – 8,8 месяцев (95% ДИ 7,3–10,6). Выживаемость без развития дистантного метастазирования на сроке 12 и 24 месяца составляет 38,9% и 24,3% соответственно.</p><p>Локальные рецидивы зарегистрированы у 72 (22,2%) пациентов. Медиана времени развития локального рецидива – 6,9 месяцев (95% ДИ 5,9–8,2).</p><p>В многофакторном анализе метастазы радиочувствительных опухолей (р=0,0027), ограниченное (≤3) метастатическое поражение головного мозга (р=0,0269) и хороший функциональный статус (р=0,0002) являются факторами лучшей выживаемости больных с МГМ</p></sec><sec><title>Выводы</title><p>Выводы. Проведение РХ пациентов с метастатическим поражением головного мозга обеспечивает в целом по группе медиану общей выживаемости 8,6 месяцев, что превышает аналогичный показатель у больных RPA 1 класса (7,1 месяц), получивших облучение всего головного мозга в самостоятельном варианте лечения. Наибольшее увеличение медианы общей выживаемости достигается у пациентов с хорошим функциональным статусом, ограниченным метастатическим поражением головного мозга и метастазами радиочувствительных опухолей.</p></sec></abstract><kwd-group xml:lang="ru"><kwd>метастазы в головной мозг</kwd><kwd>радиохирургия</kwd><kwd>интракраниальные рецидивы</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Белов Д.
