Прирост числа впервые выявленных случаев составил 63,9 % (с 83 случаев в 2013 до 136 случаев в 2023 г.).</p></sec><sec><title>Цель исследования</title><p>Цель исследования: Анализ клинико‑морфологических параметров и оценка прогноза заболевания у молодых женщин в возрасте до 40 лет, больных РМЖ на территории Красноярского края.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: Представлен ретроспективный анализ данных, полученных из Популяционного ракового регистра, Онкорегистра ТФОМС и МИС qMS, характеризующих больных РМЖ в возрасте до 40 лет в Красноярском крае за 2013–2023 гг. Статистический анализ проводился с использованием программных продуктов Microsoft Excel, Statistica 12 и StatTech 4.0.6. Анализ выживаемости проводился с использованием кривых Каплана‑Мейера, для оценки различий в выживаемости использовался лог‑ранговый тест. Исходами, представляющими интерес, были бессобытийная выживаемость (БСВ) и общая выживаемость (ОВ). БСВ определялась как период времени от диагноза до рецидива заболевания, прогрессирования заболевания или смерти. ОВ определялась как период времени от постановки диагноза до момента окончательного анализа (31.03.2024) или смерти. Уровень значимости критерия считался статистически значимым при р &lt; 0,05.</p></sec><sec><title>Результаты</title><p>Результаты: На территории края среди молодых женщин в возрасте до 40 лет преобладают случаи выявления РМЖ на ранних стадиях (73,0 %), по морфологическому типу преобладают инвазивные карциномы (89,6 %), по молекулярно‑биологическому подтипу — HR(+) Her2(–) (50,6 %). Пятилетняя выживаемость за исследуемый период увеличилась на 48,1 %, одногодичная летальность снизилась на 81,7 %. ОВ и БСВ для пациентов с поздними стадиями РМЖ статистически значимо меньше по сравнению с ранними стадиями. Корреляции между ОВ и БСВ в зависимости от подтипа опухоли не наблюдается. Более низкие показатели ОВ у пациентов с HR(–) Her2(–) в сравнении с другими подтипами. Более низкие показатели БСВ у пациентов с HR(+) Her2(–), чем у пациенток с остальными молекулярно-биологическими подтипами.</p></sec><sec><title>Выводы</title><p>Выводы: Результаты проведенного анализа показали, что рак молочной железы у молодых женщин более чем в четверти случаев (27,0 %) встречается на поздних стадиях, большой процент агрессивного молекулярно‑биологического подтипа HR(–) Her2(–) — 14,2 %. Влияние этих факторов на ОВ и БСВ говорит о необходимости наблюдения за молодыми женщинами, не входящими в скрининговую программу по раннему выявлению РМЖ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: Over 10 years from 2013 to 2023, the number of new cases of breast cancer (BC) among women under the age of 40 increased in the Krasnoyarsk region. The increase in new cases was 63.9 % (from 83 cases in 2013 to 136 cases in 2023).</p></sec><sec><title>Aim</title><p>Aim: The analysis of clinical and morphological parameters and assessment of the prognosis of the disease in young women under 40 years old with breast cancer in the Krasnoyarsk region.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: We performed a retrospective analysis of data from the Population Cancer Registry, the TFOMS Oncology Registry and MIS qMS on breast cancer patients under 40 years diagnosed in the Krasnoyarsk region for the period of 2013–2023. Statistical analysis was carried out using Microsoft Excel, Statistica 12 and StatTech 4.0.6 software products. Survival analysis was performed using Kaplan-Meier curves, and the log-rank test was used to evaluate differences in survival. Outcomes of interest were event-free survival (EFS) and overall survival (OS). EFS was defined as the time period from diagnosis to disease relapse, disease progression, or death, whichever occurred first. OS was defined as the time period from diagnosis until final analysis (3 / 31 / 2024) or death, whichever occurred first. The significance level of the test was considered statistically significant at p &lt; 0.05.</p></sec><sec><title>Results</title><p>Results: In the region, women with breast cancer were detected in the early stages predominately (73.0 %), mostly invasive carcinomas by morphological type (89.6 %) and HR(+) Her2(–) molecular genetic subtype (50.6) %. Five-year survival rate during the study period increased by 48.1 %, one-year mortality decreased by 81.7 %. OS and EFS for patients with advanced breast cancer are statistically significantly lower. There was no correlation between OS and EFS by tumor subtype. Lower OS rates in patients with HR(–) Her2(–).
