Однако четкий алгоритм принятия решения по выбору предпочтительного метода не определен.</p><sec><title>Цель</title><p>Цель: Задачей исследования явилась оценка роли индукционной химиотерапии в комплексном лечении пациентов с местно-распространенным плоскоклеточным раком ротоглотки, гортаноглотки и гортани.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: Проведен ретроспективный анализ данных реальной клинической практики об эффективности индукционной полихимиотерапии (ИПХТ) по схеме DCF перед лучевой терапией по сравнению с самостоятельной химиолучевой терапией (ХЛТ) у пациентов с III–IVa стадиями плоскоклеточного рака ротоглотки, гортани, гортаноглотки.</p></sec><sec><title>Результаты</title><p>Результаты: В исследование включено 176 пациентов (84 в группе ХЛТ, 92 в группе ИПХТ) с местнораспространенным плоскоклеточным раком головы и шеи, получавших лечение с 2022 по 2023 год. Группы не имели статистически значимых различий при сравнении по полу, локализации первичной опухоли, стадии Т, при этом в группе ИПХТ возраст пациентов был моложе, чем в группе ХЛТ (р = 0,004), стадия N2–3 чаще встречалась в группе пациентов, получавших индукционную терапию по сравнению с группой ХЛТ (р = 0,03). Частота объективного ответа (ЧОО) в группе ИПХТ после завершения DCF составила 72% (56/78), полных ответов — 15% (12/78). После завершения последующей лучевой или химиолучевой терапии ЧОО возрастала до 82% (51/62), из них полных ответов 53% (33/62). В группе ХЛТ ЧОО достигала 76% (42/55), полных ответов было 53% (33/62). Медиана длительности наблюдения составила 8,7 месяца. Медиана времени без прогрессирования в группе ХЛТ составила 7,6 месяца против 9,4 месяца в группе ИПХТ (ОР 0,67, 95% ДИ 0,43–1,04, р = 0,07). Шестимесячная общая выживаемость составила 65,7% и 92,8% для группы ХЛТ и DCF соответственно (р &lt; 0,01). После уравновешивания групп методом псевдорандомизации по основным факторам прогноза подтверждено преимущество ИПХТ + ХЛТ над ХЛТ — шестимесячная общая выживаемость в группах ХЛТ и DCF составила 66,8% и 97,7% соответственно (ОР = 0,28, 95% ДИ 0,13–0,62, р &lt; 0,01).</p></sec><sec><title>Заключение</title><p>Заключение: Результаты исследования позволяют сформировать гипотезу о преимуществе ИПХТ по схеме DCF в количестве 3 циклов с последующей лучевой или химиолучевой терапией над самостоятельной ХЛТ в отношении выживаемости без прогрессирования или общей выживаемости при местнораспространенном раке ротоглотки, гортани и гортаноглотки.</p></sec></abstract><trans-abstract xml:lang="en"><p>Treatment strategy of advanced unresectable head and neck squamous cell cancer (HNSCC) is limited to induction chemotherapy (ICT) followed by (chemo) radiotherapy (cRT) and concurrent chemoradiotherapy (CCRT). However, the role of ICT remains controversial considering the lack of overall survival benefit.</p><sec><title>The aim</title><p>The aim: to evaluate the role of induction chemotherapy in the treatment of locally advanced HNSCC.</p></sec><sec><title>Methods</title><p>Methods: We performed a retrospective clinical study that included patients (pts) with unresectable stage III–IVa cancer of larynx, oropharynx and hypopharynx.</p></sec><sec><title>Results</title><p>Results: A total of 176 pts were enrolled, 84 (48 %) received CCRT with cisplatin or carboplatin and 92 (52 %) — ICT followed by cRT. Groups were well balanced by sex, tumor site, T-stage, while in group of ICT there was a significant prevalence of younger pts (p = 0.004) and pts with stage N2–3 (p = 0,03).</p><p>The objective response rates (ORR) were 76 % (42 / 55) in CCRT arm (including 53 % (12 / 62) of complete responses) and 72 % (56 / 78) in ICT arm with complete response rate of 15 % (12 / 78). The ORR in ICT arm increased after the completion of CCRT up to 82 % with 53 % (33 / 62) of complete responses. The difference was not statistically significant (p = 0,7).</p><p>With a median follow-up duration of 8.7 months median progression-free survival (mPFS) was 7.6 and 9.4 months (Hazard ratio (HR) 0.67, 95 % CI 0.43–1.04); 6month OS was 65.7 % and 92.8 % in CCRT and ICT groups, respectively (HR 0.51, 95 % CI 0.30–0.85, р &lt; 0.01).</p><p>Propensity score matching analysis proved the 6month OS in the ICT + cRT group to be significantly longer than in CCRT group — 97.7 % vs 66.8 %, respectively (HR = 0.28, 95 % CI 0.13–0.62, p  &lt; 0.01).</p></sec><sec><title>Conclusions</title><p>Conclusions: ICT + сRT resulted in improvement of PFS and OS compared to CCRT in pts with locally advanced HNSCC.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>индукционная химиотерапия</kwd><kwd>химиолучевая терапия</kwd><kwd>плоскоклеточный рак головы и шеи</kwd></kwd-group><kwd-group xml:lang="en"><kwd>induction chemotherapy</kwd><kwd>concurrent chemoradiotherapy</kwd><kwd>head and neck cancer</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Работа выполнена при поддержке Автономной некоммерческой организации «Московский центр инновационных технологий в здравоохранении» (грантовое соглашение 2112–10/22).</funding-statement><funding-statement xml:lang="en">The work was carried out with the support of the Autonomous NonProfit Organization “Moscow Center for Innovative Technologies in Healthcare” (grant agreement 2112–10 / 22).</funding-statement></funding-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Состояние онкологической помощи населению России в 2022 году.
