Это связано с невысокой точностью лучевой диагностики. Новая система оценки лимфатических узлов Node-RADS может позволить более точно отобрать пациентов для неоадъювантной химиотерапии. В настоящее время надежно обоснована эффективность адъювантной химиотерапии при N1 в течении трех месяцев, при N2-B течение 6 месяцев, при NO-только при наличии факторов негативного прогноза. Это позволяет исследовать применимость проведения неоадъювантной химиотерапии при высокой достоверности поражения лимфатических узлов по данным компьютерной томографии в предоперационном периоде.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: Определить диагностическую эффективность применения системы Node-RADS для прогнозирования риска метастатического поражения лимфатических узлов при раке ободочной кишки.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: Проведен ретроспективный сравнительный анализ данных компьютерной томографии и патоморфологического исследования 75 пациентов, оперированных в МКНЦ им. А.С. Логинова в период с 2021 г. по 2023 г. по поводу рака ободочной кишки. Клиническая стадия рутинно устанавливалась по результатам компьютерной томографии грудной клетки и брюшной полости с внутривенным контрастированием. У этих же пациентов проведено рестадирование опухолевого процесса по системе Node-RADS с балльной оценкой лимфатических узлов от 1-5 баллов. Результаты двух методов лучевой диагностики были сопоставлены с данными патоморфологического исследования операционного материала.</p></sec><sec><title>Результаты</title><p>Результаты: Точность (AUC) компьютерной томографии при рутинном методе диагностики составила 0,69. При оценке по Node-RADS лимфоузлы от 4-5 баллов расценивались как пораженные. При статистическом анализе точность такого метода значительно повышалась (AUC0,93).</p><p>Для определения различий между результатами рутинной оценки, Node-RADS и заключения патоморфологического исследования были выявлены статистически значимые (p &lt; 0,0001) различия рутинной оценки и результатов патоморфологии, что соответствовало невысокой диагностической ценности. В то же время при сравнении результатов оценки по Node-RADS и патоморфологического исследования статистически значимых различий выявлено не было (р = 0,109), что соответствовало достаточной диагностической ценности оценки метастатического статуса регионарных лимфоузлов по критериям Node-RADS.</p></sec><sec><title>Выводы</title><p>Выводы: Система Node-RADS, представленная, как универсальная система оценки лимфатических узлов при злокачественных заболеваниях, по результатам ретроспективного анализа показывает достаточно высокий диагностический потенциал.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: Recent studies of neoadjuvant therapy for colon cancer indicate a significant percentage of patients’ retreatment, more likely due to the low accuracy of radiation diagnostics. The new Node-RADS lymph node assessment system can allow for more accurate selection of patients for neoadjuvant chemotherapy. Currently, the efficiency of adjuvant chemotherapy has been reliably substantiated for N1 within three months, for N2 within 6 months, for NO-only in the presence of negative prognosis factors. This allows us to study the applicability of neoadjuvant chemotherapy with high reliability of lymph node involvement according to computed tomography data in the preoperative period.</p></sec><sec><title>Study objective</title><p>Study objective: To determine the diagnostic efficiency of the Node-RADS system for predicting the risk of metastatic lymph node involvement in colon cancer patients.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: A retrospective comparative analysis of computed tomography and pathomorphological examination data was performed for 75 colon cancer patients operated on at the M. N. A. S. Loginov in the period from 2021 to 2023. The clinical stage was routinely established based on the results of computed tomography of the chest and abdomen with intravenous contrast. Restaging was performed in the same patients according to the Node-RADS system with a lymph node score of 1-5 points. The results of two radiation diagnostic methods were compared with the data of the pathomorphological examination of the surgical material.</p></sec><sec><title>Results</title><p>Results: The accuracy (AUC) of computed tomography in the routine diagnostic method was 0.69. According to Node-RADS assessment, lymph nodes of 4-5 points were considered involved. Statistical analysis showed that the accuracy of this method increased significantly (AUC0.93). To determine the differences between the results of routine assessment, Node-RADS and the pathomorphological examination results, we found statistically significant (p &lt; 0.0001) differences between the routine assessment and the results of pathomorphology, which corresponded to a low diagnostic value.
