В каждом пятом случае выявляется гиперэкспрессия HER2/neu, что является неблагоприятным прогностическим маркером и ассоциируется с более агрессивным течением опухолевого процесса. Однако активное внедрение таргетной терапии за последние 20 лет позволило изменить естественную историю этого заболевания. В тоже время общепринятые режимы неоадъювантной химиотерапии в комбинации с таргетной терапией сложно назвать оптимальными в виду их неудовлетворительной переносимости. Очевидно, что существует определенная категория пациенток, не нуждающаяся в назначении столь токсичных схем лечения. В данной обзорной статье мы хотим представить свой взгляд на возможность де-эскалации объема неоадъювантной терапии у больных с HER2-позитивным раком молочной железы.</p></abstract><trans-abstract xml:lang="en"><p>Breast cancer is the most common female cancer and cause of death from cancer in women worldwide. Overexpression of HER2 / neu, which is an unfavorable predictor associated with a more aggressive course of the disease, is detected in every 5th breast cancer patient. Over the past 20 years, targeted therapy has changed the natural course of breast cancer. However, standard neoadjuvant chemotherapy regimens in combination with targeted therapy are poorly tolerated and, therefore, are considered suboptimal. Evidently, not all patients require such toxic treatment regimens.
