Брюшно-анальная резекция - это последняя из возможных органосохраняющих операций, которую можно выполнить при среднеампулярном раке прямой кишки. Применение данной операции обосновано для пациентов с протяженностью профиля анального сфинктера в покое более 5 см при опухоли 7 – 8 см по данным жесткой ректоскопии; в случаях, если для соблюдения необходимой латеральной и дистальной границы резекции отсутствует адекватный визуальный контроль и высока вероятность повреждения тазовых анатомических структур, а также будет нанесена травма опухоли из-за значительной тракции кишки; в случаях, если происходит сверхрадикальное удаление прямой кишки в дистальном направлении с удалением внутреннего сфинктера.

Данный вариант операции не является органосохраняющим и пациент обречен на пассивную форму анального недержания.
